Cases of diseases shall be reported by the physician or other health care provider and by other persons either treating or having knowledge of a reportable disease, including, but not limited to coroners, persons in charge of hospitals or other institutions licensed by the Department (or their designees), persons in charge of schools (including school nursing staff), licensed daycare centers, or any other person providing testing and/or counseling to a person with a sexually transmitted infection.
